24 HOUR NOTICE OF CANCELLATION OR YOU MAY BE SUBJECTED TO
THE 3 HOUR MINIMUM.

IF USA IS NEEDED, THERE COULD BE A CHARGE TO AND FROM JOB AT
THE LABOR RATE OF $35.00 HR.

*NOTE:  USA REQUIRES A 48 HOUR WINDOW FOR LOCATING
UNDERGROUND UTILITIES.  PLEASE KEEP THIS IN MIND WHEN
SCHEDULING YOUR WORK.

IF POSSIBLE, PLEASE E-MAIL A COPY OF THE SITE PLANS AND/OR
DESCRIPTION OF THE WORK TO BE DONE.

A charge of $25.00 for each tooth replaced while drilling into unusually hard
ground.

A designated person responsible for the job will determine the diameter, depth
and the exact location of hole(s) to be excavated.

Not responsible for the following items:

Dirt/material removal from job site unless prior arrangements are made.

Incomplete or omitting hole excavations due to soil conditions, obstructions,
site access, water tables, underground utilities, or any situation beyond our
control.  The full charge of the job agreement will be applied.

Damage to property, concrete, or existing ground structures while gaining
access to hole locations for excavation due to truck tires.

Delay of job due to other crews, inspection, equipment conflict, site
accessibility, site readiness or any situation not allowing the job to be
performed at the agreed upon time.  A stand-by charge of $125.00 an hour
will be applied.

Securing hole(s) after excavation for safety issues.  See your local ordinates
or inspection department for requirements.  Recommend covering holes
with steel plates, pallets or other firm solid materials to prevent a fall into the
hole.  Recommended for added protection, cones, barricades, and staking
around the perimeter with caution tape.
